The population of NHS Surrey Heath CCG is ageing - particularly among over 85s - which will create a significant increase in long term conditions and demand for services.

The CCG has set ambitious targets as part of the Better Care Fund and aims to:

  • Prevent an increase in the number of people permanently admitted to residential and nursing home care despite the ageing population
  • Increase by 7.1% the number of older people who are still at home 91 days after discharge from hospital into reablement/rehabilitation
  • Reduce by 3.2% delayed transfers of care from hospital.

Surrey County Council has recently launched Adult Social Care Online; a suite of online checklists and self-assessments to provide residents with alternative ways to access support and information. The tool is designed to tell people if they are likely to qualify for social care support.
