The Clinical Commissioning Groups (CCG) responsible for NHS services in Surrey Heath, Ash Vale, Farnham and North East Hampshire want to improve local 'Urgent Care' services by improving the interface between NHS 111 and GP out-of-hours so they to work together, providing a higher quality of care for local people.

If your GP is based in the following areas, it will affect you:

  • Aldershot
  • Ash Vale
  • Bagshot
  • Camberley
  • Farnhborough
  • Farnham
  • Fleet
  • Frimley
  • Lightwater
  • Yateley.

The aim is to ensure the voices of patients and carers are at the heart of decision making. The questionnaire below asks about your experience of NHS 111 and GP out-of-hours care. Your responses will help us to shape how these services should work in the future.
