At some point in our lives most of us will or have come across a situation where we or someone we love has been diagnosed with a serious illness, disability or terminal illness.

If it happened to you have you ever thought about where you would like to be cared for, what treatments you would and wouldn’t want, who would look after you or speak on your behalf if you weren’t able to?

There are two useful booklets. One is a NHS booklet ‘Planning for your future: A guide’ – this gives you information and background on the questions posed above and the options that you may wish to consider.

The other booklet is our pdf Advance Care Plan (554 KB) , for you to record and keep.  It is not a legally binding document but more of a place to help guide you during your illness.
