What are Adult Community Services?

Adult Community Services help to provide the right support to people in their own home or as close to it wherever possible. A wide range of health care professionals including nurses and therapists coordinate and deliver care in the community, working with other professionals including GPs and social workers.

Adult community services can be broken down into four key areas:

  • Community nursing – community matrons and district nurses etc.
  • Therapy services – occupational therapist etc.
  • Specialist practitioners – diabetes nurse specialist etc.
  • Community hospitals – short stay in Farnham hospital or outpatient physiotherapy etc.

Additionally community health provides preventative and health improvement services, often with partners from local government and the third sector.

Background to the procurement

The Surrey Heath CCG population is expected to grow by 5% in the next 10 years, with our older demographics being higher than the national average. For health, care and support to be ‘integrated’, it must be person-centred, coordinated and delivered in the right place.

The CCG is committed to engaging with and informing its stakeholders, including GPs, partner organisations, patients and the public in its development of services.  The CCG is seeking public feedback which will help inform our thinking as we plan for a new community services contract which will come into effect from 1st April 2020.  

The CCG is aiming to provide a community service which feeds seamlessly into other local services, placing you at the centre of your care.

Our priorities and what we want to achieve

  • We want to place greater emphasis on integrated care, building on the success of our work to date
  • We want to provide people with a better experience, access and co-ordination of care
  • We want to reduce variation in health and wellbeing outcomes and inequalities in Surrey Heath
  • We want to empower people and communities through information, education and advice
  • We want to personalise care so it can flex and change with your needs
  • We want to help people to stay healthier for longer
  • We want to provide one front door for people to access health and social care in Surrey Heath

We need to continue to meet the needs of our residents by maintaining quality and reducing duplication, all whilst getting the best value from the public purse. We want your help to ensure we are providing the right services for you.

We are seeking your views to help us better understand what is important to you, what is working, and what you’d like to see change.
